YOU Can Help!! |

| 1) VICaP, our Volunteer Interfaith Caregivers Program, lets you use your talents
and people skills to help a homebound or disabled adult remain in their own home
as long as possible. YOU decide
when you want to volunteer, what service you will provide, and where you want to serve.
After
attending a two-hour orientation, you will decide which of the following
services you wish to provide:

| 2) Provide
leadership and concrete help by joining a committee. Each Center D.O.A.R.
program has an advisory committee. In
addition, we have a Public Relations Committee, Poinsettia Tea Planning
Committee, Silent Auction Planning Committee, Benefit Concert Planning
Committee, and Focus Conference Planning Committee. |

Come to our office and help with a wide variety of projects, including bulk
mailings, data entry, quality assurance calls to the people we serve, and more.
Volunteer once per week, once per month, or as your schedule permits!

4) Center D.O.A.R.’s services are provided free of charge. Generous community
support is essential to continue our vital work. Please consider making a
tax-deductible donation. You may mail a check made payable to Center
D.O.A.R., or mail the following information: Your name, credit card type
(Visa and MasterCard are accepted), account number, expiration date, amount
of gift authorized, and your signature. Our address is Center
D.O.A.R., 555 West Glendale Avenue, Phoenix, AZ 85021. Including Center D.O.A.R. in your
will, trust, or gift annuity will also help ensure continuing service to
